You don't have to use this form. A phone call, email or letter is enough. You only need to send it before your 14 days are up.

Model cancellation form

To NBR Mobility Ltd, trading as www.usedmobility.uk, Gilray Road, Diss, IP22 4EU, hello@usedmobility.uk:

I/We [*] hereby give notice that I/We [*] cancel my/our [*] contract of sale of the following goods [*]/for the supply of the following service [*],
Ordered on [*]/received on [*],
Name of consumer(s),
Address of consumer(s),
Signature of consumer(s) (only if this form is notified on paper),
Date

[*] Delete as appropriate.

How to fill it in

If you're cancelling on your own, cross out "We" and "our". If you're cancelling with someone else, cross out "I" and "my".
You're cancelling a machine, so cross out "for the supply of the following service". This also cancels any engineer set-up you booked.
Under "the following goods", write the machine's name and your order number.
Write the date you ordered, the date the machine arrived, or both.
You only need to sign it if you're posting it.
